Case Management

Targeted Case Management services are used to assist individuals in finding and obtaining psychiatric, medical, social, and educational services and supports. We connect our youth and families to Social Determinants of Health such as housing, food, transportation and interpersonal violence at discharge, transitions to another level of need or in aftercare.
