Care Coordination

We obtain your authorization to coordinate your care with your medical provider(s) such as your primary care physician (PCP) or any specialist from which you are receiving medical treatment.  This coordination of care promotes wellness and helps us customize your treatment appropriately.

After Care Services: We follow up with our youth and families to track their progress and promotion of health and safety.
