Evaluation & Assessment Services
In order to determine medical necessity we may have to conduct a diagnostic assessment using the DSM 5 to determine your person centered treatment plan. We will conduct an initial 60-90 minute intake and determine if a mental health assessment or substance use evaluation is required. We will also use screening tools to determine the severity of your healthcare needs. The comprehensive clinical assessment will be used to develop a treatment plan with you and to connect you to external resources to maximize your full potential of wellness.
